Dance Class Participation Agreement & Liability Waiver

By signing this form, I, the undersigned participant (or parent/guardian if under 18), acknowledge and agree to the following terms regarding participation in any dance classes, rehearsals, performances, workshops, private lessons, competitions, or events organized by JYZ DANCE / ZD ENTERTAINMENT LLC (“the Studio”).

1. Assumption of Risk

I understand that dance involves physical activities such as stretching, lifting, partner work, jumping, and rapid movement that may cause injury. I voluntarily assume all risks associated with participation.

I understand instructors may provide physical guidance for correction. If I do not consent, I will notify the Studio in writing.

2. Waiver and Release of Liability

I waive, release, and discharge ZD ENTERTAINMENT LLC, its owners, directors, instructors, staff, independent contractors, landlords, facility providers, and affiliated organizations from any liability, claims, or demands arising from injury, illness, property loss, or damages during or after participation — including those caused by negligence.

3. Health and Safety

I confirm that I (or my child) am physically able to participate and have no medical condition that would make participation unsafe, and I agree not to attend if I am injured, sick, or advised by a medical professional not to participate.

4. Emergency Medical Authorization

I authorize the Studio to obtain emergency medical care if necessary and accept full responsibility for any related costs.

5. Conduct and Property Responsibility

I agree to follow all rules and instructor direction. I am responsible for any damage I (or my child) cause to Studio property or equipment.

6. Media Release

I grant the Studio permission to use photos or videos of me (or my child) for promotional purposes without compensation.

7. Tuition & Refund Policy

Payments are non-refundable except as required by law.

Missed classes are not automatically credited, and makeup classes are provided only when available and approved by the Studio.

8. Binding Agreement

This release applies to all current and future Studio activities until revoked in writing.

This agreement is binding upon the participant, heirs, and legal representatives.

Course Enrollment Guidelines

1. Enrollment

Before each new semester begins, please select your classes according to the “Course Registration Form,” paying special attention to class times and locations.

2. Tuition Payment

(1) Tuition must be fully paid before the first official class. Please keep your payment record or screenshot for reconciliation.

(2) Payment options: Cash or Zelle transfer to z10097534@gmail.com

3. Class Formation

If a class has insufficient enrollment, the studio reserves the right to cancel or merge the class. Students with customization needs may discuss alternative arrangements with the studio.

4. Trial Class

(1) The first session of each course in a new semester may serve as a trial class. If the student chooses not to continue, only the trial class fee will be charged and the remaining tuition will be refunded. Under normal circumstances, once the second class has been taken, tuition is non-refundable for the entire semester.

(2) New students are welcome to take trial classes for any dance style. Trial class fees are the same as walk-in rates.

(3) If a new student officially enrolls for the full semester after trialing, the trial class becomes free and the fee paid will be credited toward tuition.

5. Walk-In Classes

Students must request and receive instructor approval at least 1 day in advance. Walk-in fees must be paid in cash before class begins. Applicable for:

(1) Returning students unable to attend the full semester due to personal reasons;

(2) Returning students wishing to try a new dance style at the last minute;

(3) New students taking a trial class.

6. Make-Up Classes

(1) Students who miss a class due to personal reasons may make up the session in another class of the same tuition level. If the make-up class has a higher tuition, the tuition difference must be paid.

(2) Make-up classes must be completed within the same semester and cannot be carried over.

(3) Please schedule with the instructor at least 1 day in advance.

(4) During make-up classes, please be mindful of regular students’ space and do not disrupt the normal class environment.

7. Rescheduled Classes

For cancellations due to weather, instructor reasons, or other force majeure:

(1) The studio is responsible for arranging a make-up class within the current semester;

(2) Students may also choose to attend another class with the same tuition level;

(3) If neither option is feasible for the student, the studio will provide a refund for the affected class.

8. Withdrawal & Refund

Withdrawal and partial refunds may be granted only under special circumstances, including medical reasons (doctor’s note required), relocating out of state, or force majeure preventing continued participation. Specific arrangements can be discussed with studio management.

Classroom Rules

1. EnrollmentThank you for choosing JYZ Dance out of passion and dedication to dance. To maintain a well-structured, efficient, and professional learning environment, we kindly ask all students to observe the following rules:

1、 Classroom Environment: Respectful, Focused, Efficient

(1) Please be on time. Change into your full dance outfit and shoes before class begins.

(2) If late, wait for a pause (during explanation or music transition) to enter the classroom, without disturbing the instructor or classmates.

(3) During class, set your phone to silent or vibrate mode to avoid distractions.

(4) Respect each student’s learning pace, do not interrupt the instructor or critique other students during class.

(5) If you need to leave early for personal reasons, notify the instructor in advance. Enter and exit the classroom quietly.

(6) During class, video recording by students is not allowed. In the last 5 minutes of class, with instructor consent, only one student may record a demonstration to share with the class.

(7) Class demonstration recordings are for class reference only. The studio retains rights to the recordings; they must not be shared externally.

(8) After-class discussions are welcome, focusing on dance techniques taught in class. Sharing insights and experiences is encouraged.

2、 Teacher Interaction: Learning-Centered and Respectful

(1) Instructor demonstrations are for teaching purposes. Please approach them professionally and refrain from inappropriate jokes or comments.

(2) Any suggestions or feedback are welcome privately to foster mutual understanding and growth.

(3) The class WeChat group, managed by instructors, is intended solely for class-related communication. Please keep discussions appropriate and on-topic.

3、 Studio Etiquette: Clean, Tidy, Orderly

(1) Keep your belongings safe. The studio is not responsible for lost items.

(2) Arrange personal items in an orderly fashion without obstructing classroom space. Keep the floor clear.

(3) Avoid spilling food or drinks that could disrupt class.

4、 Student Health & Safety:

(1) Students should assess their health before attending class.

(2) If you feel unwell or limited in physical capacity during class, inform the instructor immediately.

(3) If you have fever, cough, difficulty breathing, or other contagious symptoms that could affect yourself or others, do not attend class.

(4) The studio is not liable for accidents arising from health issues or personal negligence during class.

5、 Student External Performances: Respecting Original Work

(1) If a student wishes to perform (or compete with) choreography learned at the studio outside of studio-organized events, prior approval from the instructor is required. The student must provide the instructor with the specific date and location for record-keeping.

(2) Depending on the circumstances, cooperation or copyright fees may apply.

(3) Without instructor approval, students shall not use the instructor’s or the studio’s name for personal promotion or any unrelated purposes.

(4) If instructor coaching or collaboration is required for external performances, the student must contact the instructor separately to sign a cooperation agreement.

6、 Private Lessons: Deepen Your Dance Skill

(1) Private lessons are tailored to your needs, saving time and rapidly improving technique, rhythm, and expression.

(2) They help correct bad habits and reduce injury risk.

(3) They are beneficial for performance and competition preparation.

(4) For scheduling and fee details for private lessons, please consult with your instructor.

The studio reserves the right to modify these rules at any time and retains the final right of interpretation.
